If you have a child in need of Occupational Therapy, Physical Therapy or Speech Therapy, the first step is to call 708-524-2445. The following information is required:
- Your name
- Your phone number
- Your email address
- Your child’s name and age
- Concerns prompting the call to Kids Unlimited
- Services being requested (OT, PT, Speech, and/or Social Work)
- Your child’s doctor
- Your insurance carrier
- Your availability for treatment (days and times that are possible)
A script from your doctor is required that reads “(OT, PT, Speech) evaluation and treatment.” An ICD-10 diagnostic code is required. You can bring the script to the clinic or your doctor’s office can fax the script to 708-524-2443.
When you are contacted by the therapist who will be treating your child, you will receive additional information on intake forms required and scheduling.
We are preferred network providers only for BlueCross BlueShield PPO, although we are able to submit claims to all insurance companies. Coverage for therapy services is determined by your specific insurance policy. Please contact your insurance company to determine your coverage, out of network fees, if applicable, and if pre-authorization is required.
Parents are responsible for all therapy charges.
